This usually means no rule is turned on yet. Open Address Ninja from Apps in your Shopify admin. On the home page, the Checkout coverage tile reads Off while no rule is on. Open a rule and click Turn on.
If a rule is on but an address still passes, open the rule. The line under the rule name states which countries it checks. It also says billing addresses when the rule checks the billing address. With Only these countries or Except these countries, the rule checks only the countries you listed. A rule with the badge Needs upgrade does not run until your plan includes it.
Check the Address choice too. A rule, limit, character list, or pattern set to Billing address does not check the shipping address, and one set to Shipping address does not check the billing address. The billing check runs only when checkout has a billing address.
