This usually means no rule is turned on yet. Open Address Ninja from Apps in your Shopify admin. On the home page, the Checkout coverage tile reads Off while no rule is on. Open a rule and click Turn on.
If a rule is on but an address still passes, open the rule. The line under the rule name states which countries it applies to. With Only these countries or Except these countries, the rule checks only the countries you listed. A rule with the badge Needs upgrade does not run until your plan includes it.
