Yes, the app supports checkout extensions on Shopify Plus stores. If you are on the Growth plan and using Shopify Plus, you can add cart-based form fields directly to the checkout page.
Limitations:
Both cart-based form fields and checkout extensions are only available to Shopify Plus merchants. If you are not on Shopify Plus, these features are not available.
